In-home Tele-health program for daily patient monitoring

The in-home tele-health program is a no fee program designed for patients to utilize in their home. This technology provides daily monitoring of vital signs including blood pressure, pulse, weight, and oxygen content in the blood and transmits information to our VNA office.  Patients are also asked simple yes/no questions to assist the VNA nurse in evaluating their health. A small, easy-to-use monitor is set up in the home, and patients and family are trained to operate it.

The goal of our program aligns with the health care industry’s overarching goals to:

  • Prevent re-hospitalizations
  • Teach patients how to manage their disease states
  • Help patients comply with care plans 

The program particularly benefits individuals who are:

  • Recovering from congestive heart failure, myocardial infarction and cardiac surgery
  • Dealing with a chronic illness, such as diabetes or chronic obstructive pulmonary disease (COPD) 

Services provided include:

  • In-home installation of equipment
  • In-home training on use of equipment
  • Daily monitoring by VNA nurse of vital sign data
  • Follow-up phone calls and visits to patient when necessary
