How To Make A Referral

If your patient or client needs home health care, we can help them gain access to our in home services within 48 hours. Most often within 24 hours. Our timely intake process ensures that:

  • Your patient’s prescriptions are filled in a timely manner
  • They understand how to take and manage their medicine
  • A treatment plan is in place for continuation of care
  • The lines of communication are kept open between all care providers to make the transition home safe and supportive

When you are ready to make a referral, please gather the following information for our intake staff. We handle referrals by phone, email or fax.

  1. All patient contact information
  2. Requested start date for home care
  3. Primary care physician or referring physicians contact name and address
  4. Diagnosis
  5. Current medication list
  6. Current caregiver contact information including insurance information
